You are required to furnish the following:

  • Your limited validity passport U.S. passport;
  • One passport sized (2x2inch) photograph – see photo requirements [link to photographs] for information. Photos which do not meet the requirements will be rejected. BiH size photographs are not accepted;
  • Form DS-5504. Complete the form online, print and sign it.

Incomplete or incorrectly completed applications will be returned unprocessed.
